Are you a client of the India Street Public Health Center?
We are seeking your feedback regarding the City of Portland’s plan to consolidate social services and public health programs under one roof at 39 Forest Avenue in Portland. The goal is to make these services easier to access for clients of these programs. This consolidation will include the clinical services offered at India Street Public Health Center - The Portland Community Free Clinic, STD Clinic, and the Needle Exchange Program. Your participation in this survey will provide valuable information as staff begin planning the transition to this new space. Participation is voluntary. Thank you!
